Don’t want the booster? Here is some helpful information:
This is a resource for Medical Professionals who want to decline the booster.
We will have to wait and see how each facility responds but this is a start to hopefully help you.
“The California Department of Public Health Order requiring health care workers to receive the booster shot is contained at the following link:
You will note at paragraph 3 that “Workers may be exempt from the vaccination requirements under sections (1) and (2) only upon providing the operator of the facility a declination form, signed by the individual, stating either of the following: (1) the worker is declining vaccination based on Religious Beliefs, or (2) the worker is excused from receiving any COVID-19 vaccine due to Qualifying Medical Reasons.”
Based on this language, if you have a religious objection to the booster, we recommend sending an email to HR with the following language. These need to be submitted as soon as possible.
“Pursuant to paragraph 3. of the Health Care Worker Vaccine Requirement from the California Department of Public Health dated December 22, 2021, I decline vaccination based on Religious Beliefs.”
